This form is intended as a means for members of the public to submit a Freedom of Information Act (FOIA) request, and receive communication regarding the matter.  If you have questions regarding FOIA, please review our dedicated pages for FOIA Requests and our department's Redaction Statement  You must submit a photo both the front and back of your Arkansas issued driver's license or ID in order for your identity and residence to be verified.  Also, be mindful of the specificity of your request, and that the Batesville Police Department and City of Batesville are not required to compile information or generate previously non-existent records to satisfy a request.  Providing detailed, accurate information will ensure your request is processed in a timely manner.
